Discussing Your Medical Record With Named Representatives
As a general rule, in line with GDPR, we WILL NOT discuss a patient's medical records with anyone other than themselves, without prior consent, if they are aged 13 years or older. This could be for test results, appointment details or even your named GP. If you are happy for us to discuss any of your medical records with a named individual, a form needs completing and returning to the Practice. This can be downloaded at home by clicking here. Ensure you have correctly filled it out and signed it before returning it to us. A hard copy can also be collected from reception.
