New Patient Registration
We are continuing to accept all new registrations. Please note due to Covid19 if you require full online access to your records as a new patient to the practice, we will need ID verification prior to access. Thank you.
To complete an online application, fill out our registration form to forward on and also follow this link New Patient Registration Form. Please, where possible, email us a copy of your ID and complete the additional registration form highlighted below to send back to us.
We are happy to accept new patients moving into our practice area. To register, you need to visit reception for the forms to fill out OR you can print a copy of the GMS1 form AND the registration form by clicking the links. For children aged 0-19, please click here. Once they've been completed, please bring to reception with your required ID.
You will be asked to make an appointment for a new patient check with a clinician. This will ensure that we are aware of your medical history and any medication you are taking prior to receiving your medical notes from your previous GP.
Please ensure that you bring with you either your passport or driving licence as forms of ID when registering.
Medical treatment is available from the date of registration. Please contact reception for further information.
You can also register for online access where you can order repeat prescriptions, book appointments and request access to medical records. Please view our 'Access to Medical Records Policy' by clicking the link.
Online access for children on their parent/guardian's account ends when they hit the age of 12. If you want to reinstate this, you need to fill out a proxy form which can be downloaded by clicking here.
Discussing Your Medical Record With Named Representatives
As a general rule, in line with GDPR, we WILL NOT discuss a patient's medical records with anyone other than themselves, without prior consent, if they are aged 13 years or older. This could be for test results, appointment details or even your named GP. If you are happy for us to discuss any of your medical records with a named individual, a form needs completing and returning to the Practice. This can be downloaded at home by clicking here. Ensure you have correctly filled it out and signed it before returning it to us. A hard copy can also be collected from reception.

Please note students residing at Storthes Hall Campus should register at the University Practice.
Change of Address
If you change name, address or telephone number, please let our receptionists know by coming to surgery to fill out the form. Alternatively, click here to download it at home. If you move outside the practice area you may need to find a doctor in your new area. A list of GP surgeries in your local area can be found at https://www.nhs.uk/service-search/find-a-gp.
